We are an innovative team seeking a Director of Pharmacy to join our leadership team.
Job SummaryThe Director of Pharmacy will be responsible for overseeing medications within our member population, overseeing onsite home health infusions (acute pain management) within our travel clinical team.
Key Responsibilities:
- Responsible for covering all FDA-approved drugs for Members based on the Common Core Formulary as well as follow the Department's approved fee-for-service clinical criteria for drugs used in the treatment of opioid use disorder and pain management.
- Monitors member enrollment and pharmacy utilization, reporting out weekly to external partners, clients, and key stakeholders within the contract.
- Leads department Pharmacy utilization department meeting.
- Maintains weekly report of pharmacy utilization and authorizations per CCC Plus members to DMAS via the NCPDP Standard Format.
- Partners with appropriate vendors to facilitate Independent Assessment Certification and Coordination Team activities on behalf of each member.
- Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration. Makes recommendations for therapy changes to the physician and clinical team as appropriate.
- Maintains member database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication.
- Ensures that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated, and discussed with appropriate members of the healthcare team.
- Documents communication with physicians, nurses, other members of the healthcare team in relation to the members and their family members within the medical record via the EMR system.
- Implements policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs for the home health organization.
- Assists facility/agency staff personnel with procedures and education relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
- Bachelor's degree in Pharmacy or Doctorate in Pharmacy (PharmD)
- Active license in state of Virginia
- Experience with pharmacist verification systems
- 5+ years experience with clinical leadership role, managing risk management and safety protocols
- Prior experience with State of Virginia Medicaid, Medicare, and Board of Health
- Strong knowledge of clinical information systems (EMRs) and data analysis
