POSITION OBJECTIVE:

The Appointment Coordinator for Health Services plays a crucial role in organizing and scheduling physician-recommended clinic visits for residents. This position necessitates strong organizational abilities and effective systems management to coordinate residents' appointments with clinics, ensuring timely communication of information to Health Services staff and arranging necessary transportation. A solid understanding of developmental disabilities and the capacity to advocate for residents in a self-sufficient environment are essential. Flexibility in part-time hours may be required. This role is classified as non-exempt.


ROLE AND RESPONSIBILITIES:

The Appointment Coordinator operates under the supervision of the Director of Nursing, who reports to the Vice President of Health Services, and ultimately to the President. The role demands precise communication and documentation to support resident advocacy. This position is categorized under OSHA category II, indicating minimal exposure to bloodborne pathogens.


KEY DUTIES:

  • RESIDENT CARE AND SUPPORT: Administers medications and treatments, assists with resident transfers and direct care tasks as necessary, schedules appointments and transportation, accompanies residents to appointments, and communicates appointment details to residents and staff promptly. Addresses residents' physical needs and notifies Health Services staff or clinic personnel as required. Documents appointment information and keeps the interdisciplinary team informed.
  • REGULATORY COMPLIANCE: Follows all policies, procedures, and regulations set forth by The Phoenix Residence, Inc., including mandatory training hours and documentation of canceled appointments in medical records.
  • INTERPERSONAL COMMUNICATION: Engages effectively with residents, families, staff, health professionals, and community members. Maintains professional relationships through effective verbal and written communication, advocating for residents during clinic visits and team meetings.
  • POSITION DEVELOPMENT: Aids in training and preventative services for staff based on clinic visit outcomes. Participates in mandatory training and shares quality assurance practices as applicable.
  • SAFETY AND COMPLIANCE: Adheres to safety protocols, lifting standards, and infection control measures. Reports hazardous situations promptly.

QUALIFICATIONS:

  • High School Diploma or equivalent.
  • Current certification to administer medications or the ability to obtain it within six months of employment.
  • Preferred experience working with individuals with developmental disabilities.
  • Ability to lift or transfer residents, occasionally up to 75 lbs.
  • Capability to sit, stand, bend, reach, and maneuver wheelchairs.
  • Proficient in reading, writing, and speaking English, with strong communication skills.
  • Valid driver's license and a safe driving record for transporting residents.
  • Adequate hearing and vision to communicate effectively regarding appointments.
  • Familiarity with computers or willingness to undergo training for Health Services-related tasks.
