POSITION OBJECTIVE:

The Appointment Coordinator for Health Services plays a crucial role in organizing physician-recommended clinic visits and accompanying residents to these appointments. This role demands exceptional organizational abilities and effective systems management to align residents' schedules with clinic appointments, ensuring accurate communication of Health Services staff information and arranging necessary transportation. A strong understanding of developmental disabilities and the capacity to advocate for residents in an independent environment is essential. Flexibility in part-time hours may be required. This is a non-exempt role.


ROLE AND RESPONSIBILITIES:

The Appointment Coordinator operates under the supervision of the Director of Nursing, who reports to the Vice President of Health Services and ultimately to the President. This position necessitates precise communication and documentation to support resident advocacy. Exposure to bloodborne pathogens is categorized as OSHA category II, with minimal regular exposure to blood or bodily fluids. The role also involves recognizing and addressing residents' emotional and social needs, though it does not encompass psychotherapy.


KEY DUTIES:

  • RESIDENT CARE AND SUPPORT: Administers medications and treatments; assists with transfers and direct care as necessary; schedules appointments and transportation; accompanies residents to appointments; promptly communicates appointment details to residents and staff; addresses residents' physical needs and informs Health Services staff or clinic personnel; assists with vital signs; ensures new eyeglasses, hearing aids, or equipment are ordered as needed; documents clinic appointment information and updates ID team members as required; provides quality assurance feedback regarding clinic and professional vendors and aids in seeking services as requested.
  • REGULATORY COMPLIANCE: Complies with all policies, procedures, and philosophies of The Phoenix Residence, Inc., as well as all licensing agency regulations; fulfills mandatory training hours; documents all canceled appointments in the medical record; maintains resident medical records according to established policies and procedures.
  • INTERPERSONAL RELATIONS: Effectively communicates with residents, their families, staff, health professionals, and community members; fosters professional relationships through strong verbal and written communication; advocates for residents through telephone correspondence and efficient scheduling of clinic appointments, during clinic visits, in team meetings, nursing reports, and through documentation in medical records; notifies peers and the Direct Access Coordinator of upcoming appointments; demonstrates flexibility in scheduling to accommodate residents' needs.
  • POSITION DEVELOPMENT: Aids in training and preventative in-service for staff based on needs identified during clinic visits; shares quality assurance concepts as applicable; attends mandatory and relevant job-related in-services; educates staff on new procedures and recommendations from consultants; assists other Phoenix sites in developing systems for managing appointments and transportation.
  • SAFETY AND COMPLIANCE: Adheres to safety, lifting, and infection control protocols; follows medication administration standards; responds to hazardous situations and promptly reports potential dangers.
  • ADDITIONAL RESPONSIBILITIES: Assumes other responsibilities as required or requested.

QUALIFICATIONS:

  • High School Diploma or equivalent.
  • Current certification to administer medications or the ability to obtain within six months of employment.
  • Preferred experience working with residents with developmental disabilities.
  • Capability to lift or transfer, occasionally lifting up to 75 lbs.
  • Ability to sit, stand, bend, reach, and push/pull wheelchairs.
  • Proficient in reading, writing, and speaking English, with strong communication skills.
  • Valid driver's license and a safe driving record, with the ability to transport residents to medical appointments.
  • Adequate hearing and vision to communicate effectively about appointments.
  • Familiarity with computers or willingness to participate in training for Health Services-related projects.
