Job Summary

We are seeking a highly skilled and compassionate Nurse Coordinator to join our team at Summit Health. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of high-quality patient care and exceptional patient experience.

Key Responsibilities
  • Coordinate Patient Care: Assist physicians and staff with daily patient care coordination and physician access.
  • Communicate Effectively: Serve as a liaison between patients, staff, and physicians, ensuring seamless communication and collaboration.
  • Manage Patient Flow: Maintain efficient patient flow, prioritizing tasks and ensuring timely completion of duties.
  • Provide Direct Patient Care: Deliver direct patient care within the scope of licensure and in accordance with Summit Health policy.
  • Collect and Label Specimens: Collect lab specimens according to departmental guidelines and accurately label specimens.
  • Prioritize Tasks: Prioritize patient care by implementing physician orders and nursing procedures.
  • Prepare for Patient Visits: Ensure proper preparation for patient visits, preparing pertinent information needed for patient care.
  • Communicate Concerns: Effectively communicate problems, concerns, or issues to the Office Supervisor/Manager appropriately and promptly.
  • Verify Patient Identification: Ensure all patients are correctly identified prior to providing and/or documenting care.
  • Use EHR Effectively: Employ appropriate and timely use of Tasking in EHR.
  • Take Vital Signs: Obtain vital signs (BP, temp, pulse, height & weight) accurately and completely as required, documenting appropriately in EHR.
  • Administer Injections: Administer injections according to appropriate protocol, documenting injections accurately in EHR.
  • Triaging Calls: Appropriately demonstrate telephone triage protocols and document in EHR.
  • Schedule Appointments: Schedule appointments appropriately and accurately according to department guidelines.
  • Adhere to Safety Protocols: Appropriately adhere to universal safety precautions when administering medications and disposal of medical waste.
  • Update Medication Lists: Review and update medication lists in EHR, demonstrating understanding of prescription control and prescription refill procedures.
  • Enter Orders and Schedules: Enter orders and schedules appointments in the Practice Management System (PMS) and EHR.
  • Provide Referrals: Provide referrals and/or consult orders appropriately and in a timely manner in the PMS and EHR.
  • Maintain Logs: Check and maintain logs for emergency equipment according to policy.
  • Transition Care: Facilitate transition of care to UCC/Hospital.
  • Equipment Maintenance: Maintain proficiency with department-specific equipment, troubleshoot, and report malfunctions promptly.
Requirements
  • Education: Associate degree required, Bachelor's degree preferred.
  • Certifications: New Jersey State Nursing License and BLS certification required.
  • Experience: 0 – 1 years experience, 2-4 years experience preferred.
  • Language Skills: Ability to communicate in English, both orally and in writing.
  • Interpersonal and Organizational Skills: Strong interpersonal and organizational skills.
  • Problem-Solving and Critical Thinking: Ability to perform diverse work assignments with time limitations with a high degree of accuracy, using problem-solving, critical thinking, and priority-setting skills.
  • Technical Skills: Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred, experience with Standard Office Technology in a Window-based environment preferred, experience with patient care equipment preferred, experience with EHR, Practice Management System, and Departmental/Ancillary Systems preferred.

  • Commute to Satellite Offices: Ability to commute to satellite offices as needed.
