">Job Summary
">We are seeking a skilled Physical Therapist to join our team in Brattleboro, Vermont. As a Physical Therapist at BAYADA Home Health Care, you will be responsible for providing one-on-one care to patients in their homes, developing and implementing individualized care plans, and working collaboratively with other healthcare professionals to ensure the best possible outcomes for our patients.

- Conduct comprehensive assessments of patients' physical abilities and develop personalized care plans to address their unique needs.">
- Provide one-on-one physical therapy sessions to patients in their homes, using evidence-based techniques and technologies to promote optimal recovery and function.">
- Collaborate with other healthcare professionals, including nurses, social workers, and occupational therapists, to ensure comprehensive care and support for patients.">
- Maintain accurate and timely records of patient progress and communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.">
- Participate in ongoing education and professional development opportunities to stay current on best practices and advancements in physical therapy.">
Requirements
">To be successful as a Physical Therapist at BAYADA Home Health Care, you will need:

- A current Physical Therapist license in the state of Vermont.">
- A minimum of one year of recent work experience as a Physical Therapist.">
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.">
- Solid computer skills and prior experience with electronic medical records (EMR) preferred.">
