Signify Health is hiring board-certified nurse practitioner contractors to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

We are looking for experienced NPs who can provide high-quality patient care while working independently. As a contractor with Signify Health, you will have the opportunity to earn a competitive wage and choose your own schedule.

The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. We offer flexible scheduling options, including shifts Monday-Sunday, with no nights/weekends required.

Earn up to $2,500 to $4,000 per week (average 6-9 visits daily, base rate of $80 per completed assessment) and enjoy additional compensation via our pay-per-test model (for visits where the health plan member qualifies to receive additional diagnostic and preventive tests). Malpractice coverage, airline travel, hotel stays, and rental cars are also available.

Job Description
  • Conduct in-home visits that focus on a person's overall health and well-being and identify potential barriers to care
  • Evaluate patients' medical history and medication use
  • Provide education and support to patients and their families
  • Maintain accurate and detailed records of patient encounters
Requirements
  • Board certification as a Nurse Practitioner
  • Current state licensure
  • Strong clinical skills and attention to detail
  • Excellent communication and interpersonal skills
  • Able to work independently with minimal supervision
