Job Overview

Haven Hospice Inc. is in search of a dedicated Registered Nurse to provide compassionate care in a home setting for patients in need of hospice services.

Compensation is commensurate with experience.

Role Summary

The Registered Nurse is responsible for the planning, organization, and management of hospice care, emphasizing community health education and nursing expertise. This role requires collaboration with community resources to tailor services that address the unique needs of patients and their families.

Key Responsibilities

Patient Care Duties

  1. Conduct comprehensive assessments of patients and their families to identify hospice needs and develop care plans.
  2. Deliver professional nursing care, adhering to the standards outlined in the Nurse Practice Act.
  3. Evaluate and monitor patient conditions, adjusting care plans as necessary.
  4. Implement care plans that incorporate palliative actions, involving patients and families in the process.
  5. Administer medications and treatments as prescribed, ensuring compliance with care plans.
  6. Provide education and counseling to patients and families regarding health care needs.
  7. Assist patients with daily living activities, promoting self-sufficiency and comfort.
  8. Act as a Case Manager when designated, coordinating care for assigned patients.

Communication Responsibilities

  1. Maintain accurate clinical documentation regarding patient care and progress.
  2. Communicate effectively with physicians regarding patient needs and changes in condition.
  3. Collaborate with community health professionals to ensure cohesive care plans.
  4. Educate patients and families on self-care techniques and health management.
  5. Ensure a safe environment for patients and their families.

Additional Responsibilities

  1. Participate in on-call duties as required.
  2. Supervise and delegate tasks to ancillary staff as needed.
  3. Engage in ongoing professional development and training.
  4. Contribute to quality improvement initiatives within the organization.
Qualifications
  1. Graduate from an accredited nursing program.
  2. Current RN licensure and CPR certification.
  3. Minimum of one year of nursing experience within the last three years, preferably in hospice or home care settings.
  4. Valid driver's license and reliable transportation.
  5. Strong communication, observational, and problem-solving skills.
  6. Ability to perform physical tasks associated with nursing care.
