As a key member of our team, you will be responsible for delivering and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders.

Key Responsibilities
  • Deliver Quality Patient Care: Provide skilled nursing care to patients in their homes, ensuring their physical and emotional needs are met.
  • Document Patient Care: Accurately document all patient interactions, assessments, and care provided in a timely manner.
  • Collaborate with Healthcare Team: Work closely with the healthcare team, including physicians, therapists, and other healthcare professionals, to ensure comprehensive patient care.
  • Assess Patient Needs: Continuously assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
  • Implement Care Plans: Participate in the development and implementation of individualized care plans, ensuring patient goals and outcomes are met.
Requirements
  • Education: Graduate of an accredited school of nursing.
  • Licensure: Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Certifications: Current CPR certification.
  • Experience: One (1) year of experience required working under current nursing license.
Preferred Qualifications
  • Experience: Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home Health Experience: Previous experience in home health care.
Physical and Environmental Demands
  • Physical Demands: Must be able to lift 50 pounds, stand for prolonged periods, and move equipment without assistance.
  • Environmental Demands: Must be able to function in a wide variety of environments, including exposure to allergens and infectious diseases.
