Position Overview

Under minimal supervision, the role involves delivering ambulatory pharmaceutical care services in alignment with established protocols, collaborating closely with healthcare professionals. This includes overseeing patient medication management, providing educational resources to patients, and supervising the preparation and dispensing of medications. The focus is on promoting high-quality, evidence-based, and cost-effective pharmaceutical care within designated clinical areas.

Key Responsibilities

  • In collaboration with the Department manager or designated personnel, advocate for rational and cost-effective drug therapy for both inpatient and outpatient settings by actively participating in the decision-making process alongside physicians and other medical staff.
  • Guide and mentor Pharmacist Residents, Interns, Pharmacy Technicians, and other non-licensed staff.
  • Conduct thorough patient medication history assessments upon referrals from healthcare providers.
  • Develop comprehensive treatment plans based on diagnoses, which may incorporate drug therapy and lifestyle adjustments, ensuring smooth transitions across various care settings.
  • Identify, address, and avert potential and actual medication-related issues, including untreated conditions, inappropriate drug choices, suboptimal dosages, and adverse drug reactions.
  • Initiate, modify, monitor, and recommend adjustments in drug therapy under established protocols, utilizing information gathered from patient interviews, objective monitoring parameters, and medical records.
  • Coordinate and interpret laboratory monitoring related to drug therapy, including serum drug levels and other relevant tests.
  • Monitor patients to evaluate and enhance outcomes, drug utilization, and adherence, while minimizing adverse drug reactions and unnecessary medication use.
  • Provide patient education regarding disease states, including expected effects, side effects, the importance of compliance, and any specific instructions.
  • Ensure continuous improvement in the quality of pharmaceutical care services delivered.
  • Deliver educational sessions to providers and staff through presentations, newsletters, and individual interactions.
  • Perform additional duties as required.

Qualifications

Experience

  • Completion of a clinical pharmacy residency or equivalent clinical experience in direct patient care.

Education

  • B.S. degree in Pharmacy or Pharm.D. is required.

Licensure and Certification

  • California Pharmacist License required upon hire.
  • Basic Life Support certification from the American Heart Association.

Additional Requirements

  • Extensive knowledge of pharmacological principles, medical terminology, pathologies, and disease states relevant to medical management.
  • Proficient in aseptic techniques and the preparation of sterile IV products and chemotherapy agents.
  • Understanding of drug utilization reviews and evaluations.
  • Familiarity with Federal and State regulations governing pharmaceutical services.
  • Excellent verbal and written communication skills.
  • Ability to operate pharmacy computer systems and input prescription labels.
  • Must obtain an NPI number within one month of employment.
  • Ability to work effectively in a Labor/Management Partnership environment.
  • ACLS certification may be necessary in certain areas.
