Position Overview

Under minimal supervision, the Ambulatory Care Pharmacist delivers pharmaceutical care services in collaboration with healthcare professionals. This role involves managing patient medication regimens, educating patients, and overseeing the preparation and dispensing of medications. The pharmacist is dedicated to promoting high-quality, evidence-based, and cost-effective pharmaceutical care within assigned clinical areas.

Key Responsibilities

  • In collaboration with the Department manager or designated personnel, advocates for rational and economical drug therapy for both inpatient and outpatient settings through active involvement in the medication decision-making process alongside physicians and other healthcare providers.
  • Supervises and mentors Pharmacist Residents, Interns, Pharmacy Technicians, and other support staff.
  • Conducts thorough patient medication history assessments upon referrals from healthcare providers.
  • Develops comprehensive treatment plans based on diagnoses, which may include pharmacotherapy and lifestyle adjustments, ensuring continuity of care across various settings.
  • Identifies, addresses, and prevents medication-related issues such as untreated conditions, inappropriate drug choices, dosage errors, and adverse drug reactions.
  • Initiates, modifies, monitors, and recommends adjustments to drug therapy in accordance with established protocols, utilizing patient interviews and clinical data.
  • Coordinates and interprets laboratory tests related to drug therapy, including serum drug levels and other pertinent drug-related evaluations.
  • Monitors patients to evaluate and enhance therapeutic outcomes, ensuring effective drug utilization and adherence while minimizing adverse drug reactions and unnecessary medication use.
  • Provides patient education regarding their medical conditions, medication adherence, potential side effects, and necessary precautions.
  • Ensures continuous improvement in the quality of pharmaceutical care services provided.
  • Delivers educational sessions to healthcare providers and staff through various formats, including presentations and written communications.
  • Performs additional duties as necessary.

Qualifications

Experience

  • Completion of a clinical pharmacy residency or equivalent clinical experience in direct patient care.

Education

  • Bachelor's degree in Pharmacy or Doctor of Pharmacy (Pharm.D) required.

Licensure and Certification

  • California Pharmacist License required upon hire.
  • Basic Life Support certification from the American Heart Association.

Additional Requirements

  • In-depth knowledge of pharmacological principles, medical terminology, and disease management.
  • Proficient in aseptic techniques and preparation of sterile intravenous products and chemotherapy agents.
  • Understanding of drug utilization review processes and regulatory compliance.
  • Excellent verbal and written communication skills.
  • Ability to operate pharmacy information systems and related equipment.
  • Must obtain a National Provider Identifier (NPI) within one month of employment.
  • Ability to work effectively in a Labor/Management Partnership environment.
  • Advanced Cardiac Life Support (ACLS) certification may be required in specific areas.
