Job Title: LPN - Licensed Practical Nurse

Sunrise Senior Living is a leading provider of senior living communities, and we are seeking a skilled LPN to join our team. As a Licensed Practical Nurse, you will play a vital role in providing high-quality care to our residents.

Responsibilities:
  • Conduct regular wellness visits for residents to assess their health and well-being.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Collaborate with the Resident Care Director and other healthcare professionals to ensure seamless care delivery.
  • Monitor and report any changes in resident health status to the Resident Care Director and other relevant staff members.
  • Provide education and support to residents and their families on health-related topics.
  • Participate in quality improvement initiatives to enhance the overall care experience.
Requirements:
  • Graduate of an accredited LPN program.
  • Current state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
  • Minimum of one year of experience in a healthcare setting, preferably in a long-term care or assisted living environment.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic computer skills and proficiency in electronic medical records.
