Compensation and Benefits

The successful candidate will receive a competitive salary range of $30.55 - $38.90 per hour. In addition to a comprehensive compensation package, we offer:

Job Description

The Clinical Care Coordinator will play a vital role in ensuring the delivery of high-quality patient care. Key responsibilities include:

  • Assisting Nursing Administration with departmental leadership through the deployment of St. Ann's Mission, Vision, and Values
  • Facilitating the ongoing development of resident-specific care plans
  • Regularly communicating changes in care plans to all team members
  • Coordinating daily assignments to ensure care is provided according to established policy and procedures
  • Participating in the overall development of staff members in collaboration with the Nurse Manager and other team members
  • Collaborating with the Nurse Manager to ensure clinical operations in the household comply with federal, state, and company policies
  • Maintaining resident clinical records and ensuring confidentiality of care information
Requirements

To be successful in this role, candidates must possess:

  • RN required (dependent on organizational needs)
  • Three (3) to Five (5) years related experience/training or equivalent combination of education and experience
  • CPR Certification within one (1) month of hire and maintained thereafter
