Position Overview:

A qualified professional dedicated to delivering rehabilitation services to individuals across various age demographics. This role involves the formulation, execution, and assessment of individualized care strategies aimed at enhancing or preserving the health of patients. The specialist will design, coordinate, and implement rehabilitation programs in accordance with medical directives and patient evaluations, while collaborating with a multidisciplinary team.

Key Responsibilities:

  • Conduct comprehensive clinical evaluations to determine patient requirements and suitability for therapy.
  • Maintain accurate documentation, including the development and review of a Plan of Care (POC), progress notes, and other essential clinical records. Adjust the POC based on ongoing assessments and evaluations, working closely with physicians and the healthcare team.
  • Design and administer suitable therapeutic interventions for neuromuscular, neurological, and orthopedic conditions, guided by medical referrals and therapy assessments.
  • Educate and assist patients and their families in safe transfer techniques tailored to individual capabilities. Provide instruction on the proper use of assistive devices such as walkers, wheelchairs, canes, braces, and prosthetics, ensuring safety and effectiveness.
  • Deliver relevant patient and family education regarding therapeutic methods, considering the patient's age, condition, cultural background, and readiness to learn.
  • Supervise and guide other team members in delivering patient care that aligns with the needs and preferences of the patient.
  • Implement infection control measures and universal precautions during patient care activities to mitigate the risk of contamination and disease transmission.
  • Accept clinical responsibilities that align with professional qualifications and competencies.
  • Perform additional related tasks as assigned or requested.

Qualifications:

  • Demonstrated effective communication and interpersonal skills.
  • Familiarity with durable medical equipment.
  • Commitment to ongoing professional development and compliance with continuing education requirements.
  • Ability to adapt to a flexible work schedule and travel as needed.
  • Physical capability to perform heavy lifting, bending, pushing, and prolonged standing or walking, often under pressure to meet patient needs.
  • Manual dexterity and hand-eye coordination are essential.
  • Adherence to company values and administrative policies.
  • Proficiency in English, both written and spoken.

Required Skills:

  • Current State License as a Physical Therapist.
  • Valid CPR certification.
  • Current driver's license with reliable transportation and insurance.
  • 1-2 years of experience in a rehabilitation or acute care environment.
  • Previous experience in home care is preferred.

Educational Requirements:

Bachelor of Science in Physical Therapy from an accredited program recognized by the American Physical Therapy Association.
