We are seeking a highly skilled Per Diem Physical Therapist to join our team in Marinette, Wisconsin.
Job SummaryThis is a per diem / prn Float Physical Therapist position at Advocate Aurora Health. As a member of our therapy team, you will be responsible for providing high-quality physical therapy services to patients in need. This role involves interpreting physician referrals, conducting evaluations, and developing treatment plans tailored to each patient's unique needs. You will work closely with our interdisciplinary team to ensure seamless care coordination and effective communication.
Responsibilities- Evaluation and Treatment Planning: Interprets physician referrals and conducts comprehensive evaluations to identify musculoskeletal, neuromuscular, cardiopulmonary, and integumentary impairments. Develops individualized treatment plans based on evaluation results and functional goals.
- Patient Care and Education: Provides direct patient care, implementing evidence-based interventions to promote optimal outcomes. Educates patients and their families on proper techniques, exercises, and home programs to maintain progress between sessions.
- Clinical Documentation and Communication: Maintains accurate and timely documentation of patient progress, including progress notes, team conference reports, and discharge summaries. Communicates effectively with healthcare teams, ensuring seamless care transitions and addressing any concerns or questions.
- Professional Development and Team Collaboration: Participates in performance improvement activities to enhance quality of care and patient outcomes. Collaborates with colleagues to share knowledge, best practices, and research findings, promoting a culture of continuous learning and growth.
- Bachelor's Degree in Physical Therapy from an accredited program
- Current Physical Therapist license issued by the state of Wisconsin
- Basic Life Support (BLS) certification issued by the American Heart Association (AHA)
- Excellent communication and interpersonal skills
- Intermediate computer skills
- Demonstrated organization, decision-making, and problem-solving skills
