Job Description
We are seeking a skilled Licensed Master Social Worker (LMSW) to join our Care Coordination and Social Work Department. The ideal candidate will be responsible for facilitating inpatient hospital discharges and assessments across various settings, including sub-acute rehab, acute rehab, hospice, home discharge, and psychiatric care.

Key Responsibilities:
  • Perform comprehensive assessments and develop individualized care plans to ensure timely and effective discharge planning.
  • Collaborate with healthcare providers to coordinate referrals and insurance authorizations for various levels of care.

Required Skills and Qualifications:
This role requires an LMSW in New York State, experience with Epic and Allscripts systems, and knowledge of hospice, sub-acute rehab, long-term acute care hospitals, acute rehab, home discharge, insurance authorizations, abuse cases, and shelter admissions. Additionally, the ability to manage a caseload of 10 to 30 patients is essential.
