Care Services Navigator

Key Responsibilities:
- Deliver high-quality nursing care and coordinate hospice services for patients and their families
- Collaborate with interdisciplinary teams to develop patient-centered care plans
- Conduct regular assessments of patients' needs and preferences
- Establish and maintain strong relationships with patients, families, and healthcare providers
Requirements:
- Active Registered Nurse license in the State of Ohio without any board actions
- Two years acute care nursing experience preferred
- Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment
- CPR Certified
- Computer skills sufficient to properly document services and care
- Ability to drive during daytime, nighttime or inclement weather
- Valid Driver's License with Safe Driving Record
- State Minimum Automobile Insurance Coverage
