We are passionate about providing our patients and their families with hospice care that is rooted in the communities we serve. As a Patient Care Navigator, you will be responsible for collaboration with our Hospice Liaison team and Admissions team to increase public awareness of our services and drive admissions for patient placements.

This role will act as the first point of contact for hospitals, healthcare facilities, and other referral sources regarding referrals, networking opportunities, education, and relationship development with new and existing referral sources.

You will be responsible for identifying appropriate care and services to patients and their families in accordance with attending physician and plan of care, as well as company policy and procedures.

To succeed in this role, you must possess strong communication and interpersonal skills, with the ability to make independent decisions when circumstances warrant.
