Medical Care Coordinator

The Clinical Social Worker I plays a vital role in our interdisciplinary team, collaborating with professionals to evaluate patients' medical or physical condition and assess client needs. They work together to develop and implement treatment plans that support patient-centered care for individual patients and the medical community.
We are seeking an experienced Clinical Social Worker I to join our team. The successful candidate will demonstrate fundamental skills in formulation, assessment of risks, crisis intervention, completion of psychosocial treatment plans, and appropriate documentation.
Key Responsibilities:
- Collaborate with other professionals to evaluate patients' medical or physical condition and assess client needs.
- Maintain accurate records of client progress according to measurable goals described in treatment and care plans.
- Demonstrate basic foundational skills showing fundamental knowledge and commitment to professional values and ethics.
- Work collaboratively with all healthcare team members to develop and implement treatment plans.
- May provide assistance and guidance to new team members during onboarding.
- Use data to measure progress and promote collaboration and equity among healthcare team members.
Requirements:
- Bachelor's Degree in Social Work or Master's Degree in Mental Health & Behavioral Medicine required.
- Experience working in a multidisciplinary healthcare setting preferred.
- Fundamental skills in formulation, assessment of risks, crisis intervention, completion of psychosocial treatment plans, and appropriate documentation.
