About the RoleThe successful candidate will be responsible for providing ongoing assessment and formulation of patients' needs, updating treatment plans, and coordinating with multidisciplinary teams to ensure timely and effective care.
In addition to clinical responsibilities, this role requires excellent administrative skills, including documentation, communication, and coordination with managed care organizations.
Key Responsibilities- Provide clinical services to patients, including assessment, treatment planning, and discharge planning.
- Coordinate with multidisciplinary teams to deliver patient-centered care.
- Act as primary liaison with family members, referrers, and community agencies to develop comprehensive discharge plans.
To be considered for this role, you must have:
- A Master's degree in Social Work (MSW) and a LICSW license in the state of MA or equivalent independent Social Work licensure.
- A minimum of two years post-degree experience in an inpatient, partial, or outpatient setting.
- Excellent written and verbal communication skills, as well as the ability to work collaboratively within a rapidly changing system of care.
- An estimated salary range of $85,000 - $110,000 per year, based on experience.
