Patient Support Manager

The Elaine Ellis Center of Health is seeking a dedicated and compassionate Patient Support Manager to join our team. In this role, you will be responsible for providing comprehensive support to patients, enhancing their overall quality of care through patient advocacy, assistance, collaboration, and communication.
Job ResponsibilitiesThe ideal candidate will have a strong understanding of healthcare systems and policies, with excellent communication and interpersonal skills. You will work closely with patients, families, and caregivers to provide education, support, and guidance on navigating the healthcare system.
Key Responsibilities:
- Provide comprehensive case management services to patients, including assessments, planning, implementation, and evaluation.
- Facilitate patient follow-up with referrals and compliance with recommended appointments.
- Create a comprehensive referral base and establish professional relationships with referral specialists to ensure smooth and consistent communication.
- Coordinate chronic care management for appropriate patients.
