The successful candidate will assess patients and their families for needed assistance with resource procurement and provide ongoing support, case management, and crisis intervention as necessary.

Responsibilities
  • Provide behavioral health triage and intake assessments to determine the appropriate level of care and treatment for patients
  • Assess patients and their families for needed assistance with resource procurement
  • Provide ongoing support, case management, and crisis intervention as necessary
  • Collaborate with the behavioral health team to support behavioral services within the clinics
Requirements
  • Master's degree in social work
  • Three years of social work experience
  • LMSW or LCSW licensure with the state of Texas
  • Experience working in a healthcare setting and/or providing behavioral health services preferred
  • Bilingual English and Spanish preferred
