Job Summary

We are seeking a skilled Social Worker (MSW) to join our interdisciplinary team. The successful candidate will provide psychosocial support to patients and their families, utilizing their professional training and personal judgment to assess and address their needs.

Responsibilities
  • Assess the psychosocial needs of patients and their families, identifying areas for intervention and support.
  • Develop and implement individualized plans of care, incorporating the patient's goals and preferences.
  • Provide emotional support and counseling to patients and their families, addressing issues related to illness, loss, and death.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support.
  • Document patient interactions and progress, maintaining accurate and confidential records.
Requirements
  • Masters Degree in Social Work from a CSWE-accredited school.
  • Licensure as required by the state in which the hospice is located.
  • One year of experience as a Social Worker in a healthcare or hospice setting, with three to five years preferred.
  • Knowledge of terminally ill patients and their families, as well as the psychosocial dynamics of illness, loss, and death.
