The Emergency Room Nursing Case Specialist plays a vital role in the healthcare system by working closely with patients, their families, medical professionals, and support staff to ensure seamless patient care. This position emphasizes patient advocacy, effective communication, and comprehensive education regarding health services.
Key Responsibilities:
- Collaborate with multidisciplinary teams to facilitate optimal patient outcomes.
- Identify and coordinate necessary services, resources, and healthcare providers throughout the patient care journey.
- Conduct assessments to determine patient needs and develop appropriate care plans.
- Ensure compliance with healthcare regulations and standards.
Qualifications:
- Current Registered Nurse with a minimum of 5 years in acute medical/surgical settings and at least 2 years in case management.
- Recent acute care experience of no less than 3 years.
- Proficient in basic computer applications.
- Knowledge of case management principles, including utilization review and discharge planning.
- Possession of an active RN license in California and a BLS certification from the American Heart Association.
