About the Role

We are seeking a highly skilled and compassionate Breast Cancer Patient Navigator to join our team at e CancerCare. As a key member of our multidisciplinary team, you will play a vital role in guiding patients and their caregivers through the cancer continuum, ensuring they receive timely and optimal care.

Key Responsibilities
  • Imaging Center Duties:
    • Notify primary care physicians of diagnostic abnormalities found at St. Agnes Imaging and inform them of the multidisciplinary program available.
    • Contact patients and arrange for a breast consult prior to biopsy.
    • Follow up on biopsies and schedule appointments with breast surgeons as needed.
    • Conduct initial intake, including genetic screen tool to identify patients meeting NCCN guidelines for testing.
    • Bridge the gap between St. Agnes Imaging and the breast center.
  • cCare Duties:
    • Schedule appointments with the multidisciplinary team (med onc, rad onc) for patients with malignancy.
    • Oversee studies and additional visits required for patients, coordinating visits and testing on the same day to minimize travel.
    • Be the point person for newly diagnosed breast cancer patients to call with questions on appointments and test schedules.
    • Initiate consults to social work, nutrition, and genetics as needed.
Requirements
  • High school diploma or equivalent required.
  • Current RN license with the California State Board of Nursing.
  • Current CPR certification required.
  • Previous Breast/Patient Navigator experience preferred.
  • Strong customer service background, preferably in a healthcare environment.
  • Excellent verbal communication skills.
  • Knowledge of drug names, both brand and generic, and available dosage forms.
  • Ability to manage and prioritize multiple tasks.
  • Proficiency in computer processing functions and other standard office equipment.
  • Ability to calmly and professionally resolve customer issues with diplomacy and tact.
  • Ability to work independently with minimal supervision.
  • Strong organizational skills.
Working Conditions
  • This position functions primarily indoors in a medical/business environment.
  • Employee may be subject to blood-borne pathogens, radiation, bio waste, and other medically related factors, with exposure to moderate noise levels.
  • Visual acuity to read and compute screen and paper documents; close and distance vision, peripheral vision, depth perception, ability to adjust focus; hearing acuity to converse with staff and customers; ability to walk, stoop, bend, kneel, and lift and move 25 pounds on a regular basis; ability to sit for hours at a time.
