Job Title: Care Coordination Lead

At Northwestern Memorial Healthcare, we are seeking a highly skilled Care Coordination Lead to join our team. As a key member of our care coordination department, you will play a vital role in ensuring seamless transitions of care for our patients.

Responsibilities:
  • Collaborate with the director of care coordination to oversee the discharge planning process and operations of the case management team.
  • Utilize exceptional communication skills to build trusting relationships with interdisciplinary teams and foster an environment for shared decision making.
  • Coach and mentor case managers in their professional practice, promoting their health and well-being.
  • Collaborate with the director and department secretary to ensure a smooth onboarding process for new staff, including identifying preceptors and assigning schedules.
  • Partner with physicians to build a collaborative inter-professional practice environment.
  • Contribute to annual employee performance evaluations and provide feedback to improve employee performance and engagement.
  • Stay up-to-date with evidence-based practices and implement process improvements to enhance patient care.
Requirements:
  • Master's degree in Social Work from a CSWE accredited school of social work & Licensed Clinical Social Worker (LCSW) OR Graduate of an accredited school of nursing with a BSN.
  • Minimum of three years of case management experience in an acute care setting or healthcare environment.
