We are seeking a highly skilled and compassionate Social Worker MS to join our team at Telecare Corporation. As a key member of our multidisciplinary treatment team, you will provide clinical and casework services to individuals with severe mental illness, promoting their recovery and well-being.
Key Responsibilities- Develop and implement individualized recovery plans in collaboration with members served and their natural supports.
- Provide person-centered clinical services, including individual and group therapy, to address the unique needs of each member.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services.
- Conduct thorough assessments and develop plans of care to address the complex needs of members served.
- Document all treatment provided in a thorough and consistent manner, ensuring accurate and timely reporting.
- A Master's Degree in Social Work from an accredited graduate school and working towards licensure and registration with the Board of Behavioral Science.
- One year of direct service experience with individuals with severe mental illness in an inpatient or outpatient setting.
- Criminal justice clearance and education verification.
- Must be at least 18 years of age and possess current CPR, Crisis Prevention Institute (CPI), and First Aid certification.
- Previous experience working with the population to be served and/or in a similar program setting.
- One year of experience in a drug and alcohol setting, internship, employment, or volunteer work.
- CADAC or similar certification.
- Knowledge and experience with SBIRT and ASAM.
- Familiarity with mental health screening tools, such as GAD-7 and PHQ-9.
