Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our Medi-Cal SPD Program team at Regal Medical Group. As a key member of our team, you will be responsible for providing exceptional care coordination and social services to our Medi-Cal members, ensuring they receive the support and resources they need to achieve optimal health outcomes.

Key Responsibilities:

  • Conduct thorough psychosocial assessments of Medi-Cal members to identify their unique needs and develop personalized care plans.
  • Collaborate with our Case Management team to ensure seamless care coordination and effective management of member cases.
  • Provide education and resources to members and their families on topics such as housing, financial assistance, and community resources.
  • Develop and implement treatment plans using evidence-based modalities, including crisis intervention and community contacts.
  • Participate in Interdisciplinary Care Team meetings to discuss member care and develop comprehensive plans.
  • Consult with clinical and educational staff members to provide expertise and support.
  • Assist with hospice conversion and palliative care referrals, as well as provide resources for family and social support.
  • Document all interactions and interventions in our electronic medical documentation system.

Requirements:

  • Master's Degree in Social Work (MSW) or Licensed Clinical Social Worker (LCSW).
  • High level of knowledge of Case Management processes and principles.
  • Ability to work with diverse populations, including seniors and individuals with complex health needs.
  • Excellent communication and organizational skills, with the ability to work independently and as part of a team.
  • Proficiency in MS Office environment and EZ Cap.
  • Bilingual skills in Spanish, Farsi, Cantonese, Mandarin, Korean, or Vietnamese a plus.
