PATH is seeking a dedicated and compassionate Case Manager to join our PSH Team in Los Angeles. As a key member of our team, you will play a vital role in assisting clients in breaking the cycle of homelessness by utilizing a "whatever it takes" approach.
Key Responsibilities:
- Conduct intake and individualized needs assessments for clients, collaborating to develop Individualized Service Plans (ISPs) that address housing goals, income, and other personal objectives.
- Provide referrals, linkages, information, and support to resources that help clients achieve their ISP goals.
- Complete program-specific assessments, such as the VI-SPDAT, to evaluate strengths and challenges in addressing short-term and long-term goals.
- Maintain accurate and up-to-date client files, adhering to documentation standards and confidentiality requirements.
Requirements:
- Two years of experience in healthcare, case management, homeless services, or a related field.
- Reliable transportation, a valid driver's license, and proof of insurance and ownership for personal vehicles used during work duties.
- Ability to qualify for PATH's insurance coverage.
