Estimated Salary: $55,000 - $65,000 per year

Job Description:

Focused on bridging the gap between healthcare services and community resources, our ideal candidate will possess a deep understanding of social determinants and their impact on health outcomes.

Responsibilities include:

  • Assessing patient needs and developing individualized plans to address gaps in care;
  • Collaborating with healthcare providers to ensure timely access to necessary services;
  • Building relationships with community agencies to secure referrals and streamline processes;
  • Monitoring patient progress and adjusting plans as needed;
  • Fostering a culture of patient-centered care and compassion.

Requirements:

  • Bachelor's degree in Social Work, Sociology, Psychology, Health Education, and/or related field from an accredited college or university;
  • At least three years of experience working in a healthcare or social services setting;
  • Bilingual in English and Spanish is preferred;
  • Possess a valid driver's license and reliable transportation;
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple projects simultaneously.
