About the Role

We are seeking a highly skilled and compassionate RN Care Coordinator Manager to join our team. As a RN Care Coordinator Manager, you will be responsible for providing intermittent skilled nursing services, communicating with other disciplines, and directing nonprofessional hospice aide staff in the provision of personal care to patients.

Key Responsibilities
  • Admit patients eligible for hospice services under the physician's order
  • Report patient status and need for other disciplines to clinical leadership, attending physician, and hospice physician
  • Update care plans on an ongoing basis and complete informational visits and obtain patient consents for hospice admission per office procedure
  • Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Requirements
  • Ability to assess patient needs and formulate individualized patient care plans to meet those needs
  • Ability to work in a field setting and make sound nursing judgments
