**Job Summary**

We are seeking a skilled Home Health Physical Therapist to join our team at LHC Group. As a Home Health Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in their homes.

**Key Responsibilities:**

  • Evaluate patients' functional status and physical therapy needs, and assist in the development of a therapy plan of care.
  • Perform interventions aimed at improving and enhancing patients' well-being, and evaluate their progress.
  • Observe and report patients' responses to treatment and any changes in their condition to the supervising nurse and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating members of the healthcare team, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.

**Requirements:**

  • Current Physical Therapy licensure in the state of practice.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable transportation.
