Job Summary

We are seeking a highly skilled and dedicated Physical Therapist to join our Home Health team. As a Physical Therapist, you will be responsible for evaluating patients' functional status, developing and implementing therapy plans, and providing services within the scope of practice defined by state laws.

Responsibilities
  • Evaluate patients' functional status and physical therapy needs
  • Develop and implement therapy plans in accordance with state laws and in coordination with other healthcare team members
  • Provide services within the scope of practice and in accordance with the plan of care
  • Observe and report patients' responses to treatment and any changes in their condition
  • Treat patients to relieve pain, develop and restore function, and maintain maximum functioning
  • Instruct and inform healthcare team members, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals
Requirements
  • Current Physical Therapy licensure in the state of Michigan and Wisconsin
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
