Job Title: Home Health Physical Therapist

LHC Group is seeking a skilled Home Health Physical Therapist to join our team in Menominee, Michigan.

Job Summary:

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to patients in their homes. As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing and implementing treatment plans, and educating patients and their families on proper care and management.

Responsibilities:

  • Evaluate patient's functional status and physical therapy needs
  • Assist physician in developing therapy plan of care
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning
  • Instruct and inform participating members of the healthcare team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals

Requirements:

  • Current Physical Therapy licensure in state of practice
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable transportation
