We are seeking a skilled Registered Nurse to join our team as an Admission and Discharge specialist. As a key member of our healthcare team, you will be responsible for ensuring a smooth transition for patients coming in and out of acute care hospital settings.

In this role, you will utilize your specialized assessment skills to ensure that patients are placed in the appropriate care areas and discharged efficiently. You will also provide education and post-hospitalization care as needed, advocating for patients, families, and caregivers while embodying our organization's vision, mission, and values.

Responsibilities:
  • Obtain nursing assessment histories and ensure proper inpatient care unit admissions
  • Complete and facilitate the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation
  • Teach patients/families/caregivers about a patient's medical condition, treatment plan, medications, and possible side effects
  • Coordinate with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions

We offer a Monday-Friday schedule with no overnights, holidays, or weekends, and hours from 9:30am-6pm (with flexibility based on hospital needs).
