About the Role

We are seeking a skilled Registered Nurse for Admission and Discharge to join our team. As a Patient Care Transition Specialist, you will be responsible for:

  1. Obtaining a nursing assessment history and ensuring proper inpatient care unit admission.
  2. Taking appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
  3. Completing and facilitating the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
  4. Teaching patients/families/caregivers about a patient's medical condition, patient status, treatment plan, medications, and possible side effects, and follow-up measures.
  5. Ensuring patients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.
  6. Coordinating with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
Requirements

To be successful in this role, you will need:

  • A Graduate of an accredited school of Professional Nursing.
  • A Bachelor's degree in Nursing (BSN) or current enrollment in a Baccalaureate or Master's in Nursing Program.
  • 2+ years of experience in an acute care setting.
  • Current RN licensure by the State of Kansas or from a Compact State if you declare the Compact State your Primary State of Residence.
  • American Red Cross or American Heart Association Basic Life Support (BLS) certification.
