Job Title: Bilingual Nurse Care Manager

Medix is seeking a highly skilled Bilingual Nurse Care Manager to join our team. As a key member of our multidisciplinary care team, you will be responsible for providing high-quality care to our high-risk patients.

Job Summary:

The Bilingual Nurse Care Manager will work collaboratively with our care team to develop and implement organization-wide approaches to problem-solving, tracking, and managing complex cases and populations. This role requires a strong ability to plan effectively, identify social determinants of health, manage chronic conditions, and promote efficient resource use.

Key Responsibilities:
  • Implement care pathways for patients with chronic conditions
  • Oversee transitions of care for patients to ensure safe transitions from acute to post-acute care
  • Collaborate with all providers, care team, patients, caregivers, payers, community resources, and external providers to promote quality of care
  • Perform comprehensive assessments for physical, mental, and social risk factors that support individual patient needs
  • Coordinate and communicate with providers and care team members to ensure timely and cost-effective care
Requirements:
  • Bachelor's degree in nursing or healthcare-related field
  • Minimum 2 years of experience as a Registered Nurse
  • Minimum 2 years of experience in utilization management, case management, chronic care management, discharge planning, transitions of care management, cost/quality management program, and/or another related field
  • Spanish Bilingual
