Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team at Altoona Health and Rehab. As a LPN, you will play a vital role in providing direct nursing care to our residents, ensuring their physical, emotional, and social well-being.

Key Responsibilities:
  • Administer medications and treatments as prescribed by physicians, monitoring residents for any adverse reactions and providing education on medication management and side effects.
  • Assist with resident assessments, including collecting vital signs, monitoring symptoms, and documenting changes in condition.
  • Provide direct nursing care, including wound care, catheter care, and assistance with activities of daily living.
  • Collaborate with registered nurses, physicians, and other healthcare professionals to develop and implement individualized care plans for residents.
  • Supervise and delegate tasks to certified nursing assistants (CNAs) to ensure the delivery of safe and effective care.
  • Document nursing assessments, care plans, interventions, and outcomes accurately and timely in accordance with regulatory standards and facility policies.

Requirements:
  • Current LPN licensure in the state of employment.
  • Experience working in a long-term care or rehabilitation setting.
