About the Role

We are seeking a compassionate and skilled Pediatric Home Care Nurse to join our team at Phoenix Home Care and Hospice. As a Private Duty Nurse, you will have the opportunity to make a meaningful difference in the lives of children and young adults with specialized healthcare needs.

Responsibilities
  • Assist in the development, review, and revision of the plan of care for pediatric patients.
  • Provide nursing care in accordance with the physician's plan of treatment, individual plan of care, and authorized by the client's payer source.
  • Observe, record, and report reactions to treatment and any changes in the client's condition to appropriate personnel and/or physician.
  • Monitor and record patient's condition and document provided care services via electronic charting.
  • Provide ongoing instruction and supervision of client, family, or other care team personnel in the plan of treatment.
  • Participate in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.
Requirements
  • Active Licensed Practical Nurse or Registered Nurse License.
  • Positive, professional attitude.
  • Problem-solving skills and ability to multi-task.
  • Excellent communication skills.
