Job Description

Job Title: Pediatric In-Home Care Nurse

Job Summary:

We are seeking a compassionate and skilled Pediatric In-Home Care Nurse to provide high-quality nursing care to children in the comfort of their own homes. As a Pediatric In-Home Care Nurse, you will work closely with our team to deliver individualized care plans that meet the unique needs of each patient.

Responsibilities:

  • Deliver compassionate, individualized nursing care to patients based on their unique needs and treatment plans.
  • Assist in development, review, and revision of the plan of care.
  • Build strong emotional connections with patients and their families to offer a supportive and reassuring presence.
  • Observe, record, and report reactions to treatment and any changes in client's condition to appropriate personnel and/or physician.
  • Provide ongoing instruction and supervision of client, family, or other health team personnel in the plan of treatment.
  • Monitor and record patient's condition and document provided care services via electronic charting.
  • Participate in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.

Requirements:

  • Active Licensed Practical Nurse or Registered Nurse License.
  • Problem-solving skills and ability to multi-task.
  • Excellent Communication Skills.
  • Positive, Professional attitude.
