Key Responsibilities- Assist members in identifying and working towards achieving their personal goals, promoting empowerment and independence.
- Provide vocational support and education to help members develop skills and confidence.
- Collaborate with staff and members to develop and implement activities, programming, and groups that promote socialization and community engagement.
- Facilitate communication and cooperation among agencies and stakeholders to ensure seamless service delivery.
- Participate in medication outreach and coordination, ensuring timely and effective communication between healthcare providers and members.
- Educate community members and other agencies about mental health services and resources.
- Transport members to appointments and activities, as well as conduct field visits and medication outreach.
- Provide crisis intervention and support, ensuring the safety and well-being of members.
- Manage a caseload of clients, maintaining accurate records and completing required documentation.
- Assist members in accessing and maintaining stable housing.
- High school diploma or equivalent required; lived experience a plus.
- At least 2 years of experience providing direct mental health services; optimum qualifications involve 2-4 years of experience.
- California driver's license and current vehicle insurance/registration required.
Sunday to Thursday, 1:30 pm to 10:00 pm.
