About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a PACE Program Care Manager. This critical role involves providing comprehensive care management, coordination, and services to at-risk elderly patients with complex medical and psychosocial issues.

Key Responsibilities

  • Develop and implement individualized care plans that address patients' medical, social, and emotional needs
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
  • Provide education and support to patients and their families to promote self-care and independence

Requirements

To be successful in this role, you will need:

  • A current Oregon Registered Nurse license
  • 1 year of experience in home health nursing or a related field
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of an interdisciplinary team

Compensation and Benefits

The salary for this position ranges from $40.20 to $63.64 per hour, depending on experience. We also offer a comprehensive benefits package, including medical, dental, vision, life insurance, disability insurance, paid time off, and retirement savings plan with employer matching.
