Hospice LPN Care Specialist

We believe that the best definition of care is determined by the wishes and needs of patients and their families. Our team carries low patient caseloads and are never on call, allowing for a manageable workload and a focus on delivering exceptional care.
Key Responsibilities:
- Provide direct patient care as defined in State Nurse Practice Act
- Implement current nursing practice following a comprehensive assessment and the plan of care
- Provide accurate and timely documentation of patient services to reflect the plan of care
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
- Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group
- Provide appropriate pain/symptom management. Evaluates the patient's response to treatments/medications
Requirements:
- Graduate of an accredited practical nurse or a vocational nursing program
- Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
- Currently licensed as an LPN/LVN in the practicing state
- Must possess current CPR Certification
- Demonstrates good verbal and written communication, and organization skills
- Valid driver's license with an automobile that is insured in accordance with state requirements
- Understands the philosophy of hospice concept and the needs of the terminally ill
