Job Summary:

The Direct Support Professional will assist individuals with daily tasks such as personal hygiene, meal preparation, transportation, and social activities. You will work closely with our team to provide individualized support and promote personal growth and independence.

Responsibilities:
  • Provide transportation and supervision for individuals as required to meet employment, recreational, and leisure activities.
  • Perform inspections of vans and ensure that all training for the safe transport of residents is followed.
  • Perform housekeeping responsibilities, including kitchens, bathrooms, windows, bedrooms, and floors.
  • Maintain all standards when preparing meals and managing diets, following guidelines for safe handling of food and ensuring adequate supplies.
  • Provide personal care support to individuals, including changing, dressing, and other ADLs.
  • Complete all documentation and notes as required using the Therap system and other programs.
  • Ensure that goals are being completed according to each individual's ISP.
  • Conduct evacuation drills and record results.
  • Assist in the planning and facilitation of person-centered outings.
  • Complete behavioral tracking logs as required.
  • Follow all agency, State, and Federal laws and policies, using PolicyStat as needed to access policies.
  • Complete all required training, using Relias to either complete training or register for courses.
  • Manage medical emergencies according to the health service plan, attending doctors' appointments, transporting residents to medical appointments, and emergency rooms as required and requested.
  • Identify and report maintenance issues to the supervisor or through NetFacilities if available.
  • Report to work promptly and on time, not leaving work until the assigned shift ends or has approval from the supervisor.
  • Observe policies/procedures governing requests for annual, sick, or other leave time.
  • Complete Medication Administration and Proxy training, administering meds to residents and following all policies related to MAR entries.
  • Track vital signs and other health-related information for residents.
  • Perform all other duties as assigned.
  • Must be able to lift and move up to 50lbs with or without accommodations.

  • Proficient with Microsoft Office Suite or related software.
  • Ability to use technology to record notes.
Certifications and Licenses:

Must have a valid driver's license, CPR/FA, and CPI certifications.
