We are seeking a skilled Hospice RN Case Manager to join our team at Affinity Care of Pennsylvania. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals and families in their homes and communities.
Key Responsibilities- Conduct comprehensive assessments of patients and families to determine their unique needs and develop personalized care plans.
- Provide professional nursing care, including administering medications, treatments, and other interventions as prescribed by the physician.
- Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions between settings.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellence in care.
- Graduate of an accredited school of nursing with a current RN license in good standing.
- Minimum of one year of experience as a professional nurse, preferably in hospice or home care.
- Excellent observation, verbal, and written communication skills, as well as problem-solving and basic math skills.
- Ability to lift, position, and transfer patients, as well as lift supplies and equipment.
- Valid driver's license and reliable transportation.
