About the Role

We are seeking a compassionate and experienced Registered Nurse to join our team as a Hospice RN/ RN Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in their homes.

Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to the patient's family.
  • Act as primary liaison between physician, patient/family and hospice team.
  • Manage and collaborate with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA) and support staff to provide comfort, peace, and dignity.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
  • Maintain all assigned patient clinical records.
  • Participate in company quality assessment and performance improvement programs.
About You

We are looking for a candidate with a strong passion for hospice care and a commitment to delivering exceptional patient outcomes. The ideal candidate will have:

  • A RN license in the state of requested employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
  • Additional experience in hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry or travel nursing is a plus.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
