About Us: Bluebonnet Health Services is committed to delivering exceptional patient care in the comfort of their own homes. We are seeking a skilled Physical Therapist to join our team and provide high-quality physical therapy services to our patients.
Salary: $70,000 - $90,000 per year, based on experience.
Job Description: As a Physical Therapist with Bluebonnet Health Services, you will be responsible for developing and implementing individualized physical therapy plans for patients in their homes. You will conduct thorough assessments of patients' physical abilities, identify areas for improvement, and create customized treatment plans to address their specific needs.
Key Responsibilities:
- Assess patients' physical abilities and create comprehensive treatment plans
- Provide hands-on physical therapy interventions to patients in their homes
- Monitor patients' progress and make adjustments to treatment plans as needed
- Collaborate with patients, families, and other healthcare professionals to ensure seamless care coordination
Qualifications:
- Doctoral degree in Physical Therapy (DPT) from an accredited program
- Active licensure as a Physical Therapist in the state where you will be working
- At least one year of experience as a Physical Therapist, preferably in home health care
- Excellent communication and interpersonal skills
