Developmental Disabilities Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at SKIL Resource Center. As a Case Manager, you will work closely with individuals with developmental disabilities, their families, and support networks to provide personalized support and services.
Key Responsibilities- Assess individual needs and develop personalized support plans to promote independence and community integration.
- Coordinate and arrange for services and supports, including home and community-based services, to meet individual needs.
- Advocate for individuals on an individual and systems level to ensure access to necessary services and supports.
- Provide quality assurance of services rendered to ensure individuals receive high-quality care and support.
- Collaborate with community organizations, service providers, and other stakeholders to ensure seamless service delivery.
- A minimum of six months experience in the field of developmental services.
- A Bachelor's degree or additional experience in the field, which may be substituted for the degree at the rate of six months of experience for each semester.
- Valid Driver's License.
