Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at SKIL Resource Center. As a Case Manager, you will work closely with individuals with developmental disabilities, their families, and support networks to provide personalized support and services.

Key Responsibilities
  1. Assess individual needs and develop personalized support plans to promote independence and community integration.
  2. Coordinate and arrange for services and supports, including home and community-based services, to meet individual needs.
  3. Advocate for individuals on an individual and systems level to ensure access to necessary services and supports.
  4. Provide quality assurance of services rendered to ensure individuals receive high-quality care and support.
  5. Collaborate with community organizations, service providers, and other stakeholders to ensure seamless service delivery.
Requirements
  1. A minimum of six months experience in the field of developmental services.
  2. A Bachelor's degree or additional experience in the field, which may be substituted for the degree at the rate of six months of experience for each semester.
  3. Valid Driver's License.
  4. Clear background checks.
