Key Responsibilities:
- Provide high-quality patient care to individuals with complex medical conditions, including those with chronic illnesses such as CHF, COPD, and diabetes.
- Work collaboratively with a multidisciplinary team, including physicians, nurses, and social workers, to develop and implement comprehensive care plans.
- Conduct home visits and provide care to patients in their homes, as well as in office settings.
- Participate in shared call with high-risk providers and contribute to the development of new practice ideas and initiatives.
- Collaborate with dedicated support staff, including Care Partners and Social Workers, to ensure seamless patient care.
- Work in a dynamic and fast-paced environment, with a focus on delivering high-quality patient care and achieving exceptional outcomes.
Requirements:
- Master's degree from a four-year college and/or a professional certification beyond a four-year college.
- Certificate of completion from an accredited Nurse Practitioner or Physician Assistant Program.
- Unrestricted California Advanced Practice Clinician license.
- Current AANP/ANCC/NCCPA certification.
- Current California DEA certificate required prior to start date.
- 2+ years of experience in ambulatory care or a similar environment.
- EMR Proficient.
- Excellent patient care and time management skills.
- Ability to work in a fast-paced environment, as a team player, and adapt to changing priorities.
Preferred Qualifications:
- Critical Care, Palliative, or Geriatrics certification as a Nurse Practitioner.
- Experience in a managed care setting and working with medically complex patients.
- Fluent in Medical Spanish.
Compensation and Benefits:
The salary range for this position is $110,040 to $139,000. UnitedHealth Group offers a comprehensive benefits package, including incentive and recognition programs, equity stock purchase, and 401k contribution.
