Job Summary

We are seeking a highly motivated and organized Patient Care Coordinator to join our team at Capital Caring Health. The successful candidate will be responsible for supporting daily operations in our home care and facility settings, ensuring great patient and family experiences, and managing CNA schedules and operations for providers.

Key Responsibilities
  • Manage daily administrative functions, including scheduling and coordinating with the clinical team and executive leadership.
  • Ensure high-quality patient care by communicating effectively with patients, families, and healthcare providers.
  • Coordinate with the clinical team to ensure timely and accurate completion of patient care plans.
  • Manage CNA schedules and operations, including staffing and supply management.
  • Provide exceptional customer service and support to patients, families, and healthcare providers.
Requirements
  • 2 years of experience in a healthcare setting, with some elder health experience preferred.
  • High school degree required, Associate's degree desired.
  • Valid driver's license and access to a vehicle during working hours.
