Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at the Long Island State Veterans Home. As a Licensed Practical Nurse, you will play a critical role in providing high-quality patient care and contributing to the overall well-being of our residents.

Key Responsibilities
  • Provide direct patient care, including administering medications, performing treatments, and monitoring patient responses.
  • Develop and implement individualized care plans, taking into account the unique needs and goals of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and effective communication.
  • Document patient progress, including vital signs, medication administration, and treatment outcomes.
  • Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of evidence-based practices.
Requirements
  • Current licensure as a Licensed Practical Nurse in New York State.
  • One year of experience in a healthcare setting, preferably in a long-term care or geriatric population.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
Preferred Qualifications
  • Experience with electronic medical records (EMRs) and other healthcare technology.
  • Knowledge of geriatric care principles and practices.
  • Ability to work flexible hours, including evenings, weekends, and holidays.
